The incident comes two months after Moore split from her husband, Ashton Kutcher.

Demi Moore was rushed to a Los Angeles hospital late Monday and is entering rehab for substance abuse, the New York Daily News is reporting.
The newspaper described the actress as "painfully thin."
"Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health," her publicist Carrie Gordon said in a statement to the Daily News. "She looks forward to getting well and is grateful for the support of her family and friends."
Earlier in the day, TMZ reported a 911 call was placed at 10:45 p.m. Monday night. Paramedics responded to Moore's Los Angeles home and -- after assessing her for a half hour -- transported her to a local hospital.
TMZ said the emergency call was related to substance abuse.
The incident comes two months after Moore, 49, split from her husband of six years, Ashton Kutcher, 33. The couple's relationship became tabloid fodder as rumors swirled about Kutcher's alleged infidelity.
